2019 Centaurus High School Fall Athletic Registration Information
Boys' Golf - Registration starts July 15th, 2019.
Athletic Registration - Complete the Athletic registration by deadline.
Sports Physical - Write the name of the sport your student athlete has registered for on the top of their current sports physical then scan and email this to Centaurus High School Athletic/Activities Administrative Assistant.
Returning CHS Student Athletes - If you have a current sports physical on file from the previous school year, you do not have to submit a new physical until that one has expired.

The test is an open book test and you can take it as many times as you need.
All players need to take the test in order to participate in the Tryouts
All players need to take and pass the test in order to participate on in tournament play.

Volunteers
If you would like to participate on our summer program (i.e. play a round of golf with the students) you must register as a Volunteer.
Random people helping/working this summer
No person is allowed to coach, chaperone, interact with our kids or attend practices clinics and camps without being a volunteer or an employee! No "randoms" around our kids!* Basically, they need to be a BVSD employee or BVSD volunteer to help out this summer.
Use the attached application or volunteer forms and information to ensure our kids are safe.
No non-BVSD employees or volunteers are allowed to be alone with our students (That includes parent volunteers or former students!)*
Additionally, volunteers and newly hired coaches must watch and complete:
Concussion training video: Concussion in Sports Video (free)
Child Abuse Reporting: Report Child Abuse (watch video)

Exception: If a random person is only going to attend your event three or fewer times (e.g. a parent volunteer is coming to your open gyms only two times this summer, they don't need to do any of this BUT you must keep them in line of sight. They cannot be alone with our students.)
BVSD Volunteer Application Instructions
Volunteers now have the option to fill out BVSD volunteer forms through the BVSD Webstore! You will be able to select from the following options:
BVSD Volunteer Application (all new and returning volunteers must fill this out on an annual basis, free of charge).
CBI Background Check Application (this is a one-time application, the cost is $40, once that is approved it is valid for the remainder of the volunteer’s time at BVSD).
Driver Volunteer Form (an additional form that must be completed annually for volunteers who want to be drivers, free of charge).
Volunteers can pay for their CBI Background check online with a credit card or your bank account information! You will be prompted to do so when filling out the online application. You can also choose to pay with cash or check when you come in to the Ed Center for your fingerprinting appointment (see below).
Volunteers can now come to Ed Center to get fingerprinted! After you have filled out the online CBI Background Check Application, you will receive a confirmation email with instructions on how to schedule an appointment.
